Already mentioned.

Magazine spring too weak to lift next round during bolt cycle time.

Under powered ammo.

Gritty or lack of lubrication.

Not mentioned.

Over sprung preventing full travel of bolt.

Under sprung - bolt traveling faster than magazine can lift next round.

Under gassed:

Gas block to gas port misalignment.

Undersized gas port.

Improperly aligned gas tube.

Damaged gas tube.

Probably the mag. I have seen a ton of issues with hex mags. If you switch out and still have the same issue, Blasters list is pretty comprehensive.

I had the same issue with my other and a new hexmag.  After a bit of use, it feeds perfectly now.

I finally had a chance to go to the range to try different mags. Unfortunately it didn’t help. It cycled 3-5 rounds a few times. It mostly was a single shot. The bolt hold open would not stay open after last round so the bolt is not going back far enough. Any suggestions? Adjustable gas block maybe ?

Sounds like my friend's SR 762. Once we got the proper buffer tube, which allows the bcg to retract adequately, it now is ready to run.
He initially ran MKE ammo and it seems to be too weak for his gun, even at the largest gas setting. When running my reloads (Wolf 150g .308 Win. works okay, too), it functions fine  on the normal gas setting.
The MKE will fire, extract and eject, but not 100%. Often the bcg just doesn't retract far enough to fully feed the next round, resulting in that crushing that you're experiencing.
This may not be the proper diagnosis, but with other ammo all magazines (DPMS, which have a rather flimsy follower, IMO) the firearm has been running as advertised.

The problem was the gas port on the barrel. To small. Delta arsenal fixed it for me. How do larger gun manufactured make a firearm with a problem like that? Did they forget to change the size of a bit during manufacturing process llol. Was there a large batch that went out like this?

Several people here have said Stag's quality has suffered since it changed ownership.

Did Delta have to look for the problem, or did they already know about this issue when you brought it in?
