Go to Pearle Vision.  Buy Oakley Flak Jacket protective prescription lenses.  Profit.

Dr Colo in Suffield. He is a skeet shooting pro gun eye doctor. Awesome guy. I go to him and he actually made me bring in my shotgun when I ordered RX shooting glasses, to ensure that I was sighting properly. With regular RX glasses, shooters frequently will look out the sides or over the top of the lenses while tracking clay targets, which can cause distorting.

He also gave me some skeet tips--he shoots all over the country so he knows his stuff.

His office staff all have permits too.

Musket's guy seems ideal.
There is an operation out in Colorado that specializes in shooting glasses, can make the optical axis off-center.

I tried to get some rx sunglasses from their cycling rx operation but my prescription was too strong.  Boo.

There is a company called Hansen’s Eagle Eye Shooting Glasses that will grind a special pair of glasses for you that can give you everything you need.  If you already wear bifocal glasses for distance and reading, they will grind a special area in the lens that is the "sweet spot" that brings your front sight into clear focus when you move your head to acquire the proper sight picture.

You can then wear these glasses as your everyday set and if you ever need to present your weapon from your concealment holster, your eyes will be able to clearly identify your attacker AND focus on the front sight!

You can contact Richard Hansen, the owner of Eagle Eye Shooting Glasses at http://www.hansenseagleeye.com.

I know I am pushing up an old thread, but I hit 44 this year and now I am having problems with the front site being fuzzy not just the rear site.  Curiously, only at the indoor range, not so bad outside.  Maybe I need to eat more carrots

Anyway, I am going to get my vision checked at Costco, and then give these guys a call.  The link to the doctor in Suffield is not working and I cannot seem to find him with a search.
